Medication Therapy Management FAQ
As defined in a consensus definition adopted by the pharmacy profession in 2004, MTM is a service or group of services that optimize therapeutic outcomes for individual patients. MTM services include medication therapy reviews, pharmacotherapy consults, anticoagulation management, immunizations, health and wellness programs and many other clinical services. It provides eligible patients with the opportunity to speak with a clinical pharmacist who will work with the patient and their medical providers to optimize their medication regimen and to help patients take all the medicines on time and as prescribed. MTM clinical pharmacists will help patients and their medical providers to understand the issues and concerns you face with your medications and your health – and help you to feel your best by getting the most out of your medications.
Specifically, MTM through the RCC is an active partnership between an MTM pharmacist and a patient. Ramsell MTM pharmacists work with patients to help maximize the therapeutic benefits of a medication regimen. They accomplish this by carefully reviewing a patients medication regimen through a comprehensive clinical screening using both industry standard reviews, proprietary clinical algorithms and utilizing the clinical experience of the MTM pharmacist. They then support the patient by recommending any changes to their medication regimen and to fully assess and provide counseling and solutions to patients on their medication regimen. RCC MTM pharmacists also assess a patient's adherence to their therapies and works to identify any barriers to full adherence and to work with a patient to successfully integrate their medication regimen into their lives. Finally an RCC MTM pharmacist will then work collaboratively with a patients' medical provider and case manager to realize the necessary changes to a patients' medication regimen that are needed for a patient to have the best possible clinical outcomes.
Medication-related problems and medication mismanagement are a massive public health problem in the United States. Experts estimate that 1.5 million preventable adverse events occur each year that result in $177 billion in injury and death. Medication non-adherence is also a primary driver of medication therapy failure in the US and has been estimated to cost up to $280 billion in excess healthcare utilization annually. MTM, and the RCC, are specifically focused at working with patients to fit their medication regimens into their lives and improving patient adherence to medications.
In this era of reduced budgets, MTM and the RCC are a proven area of cost savings. The average return of investment (ROI) for most MTM program is at least 3-to-1 with many program reporting a 4-to-1 ratio or more through reduced net healthcare utilization (medical provider appointments, ED visits, hospitalizations, etc.)
Pharmacists provide MTM services in all care settings where patients take medications. While pharmacists in different settings may provide different types of MTM services, the goal of all pharmacists providing MTM is to make sure that the medication is right for the patient health conditions and that the best possible outcomes from treatment are achieved.
For patients enrolled in the RCC, MTM services are provided by experienced clinical pharmacists based out of Ramsell Pharmacy Solutions home office. RCC MTM pharmacists work with patients via the telephone, mail and email on a regular basis and provide follow up and to medical providers and other interested parties via mail, telephone, fax and email. In the near future, RCC enrolled patients may have the additional option of receiving MTM services from their local pharmacy
Anyone who uses prescription medications, non-prescription medications, herbals, or other dietary supplements may potentially benefit from MTM services. People who may benefit the most include those who use several medications, those who have several health conditions, those who have questions or problems with their medications, those who are taking medications that require close monitoring, those who have been hospitalized, and those who obtain their medications from more than one pharmacy.
The RCC is designed to meet any MTM need for both Part D and specialty patient services. The RCC is fully compliant with all Medicare Part D regulations. As a flexible and scalable system, the RCC can be tailored to meet almost any specialty patient needs. Current specialty patients include those infected with HIV and Hepatitis B and C.
The RCC provides all CMS mandated forms of MTM screening and follow up. In addition, customized and scaled interactions and follow ups can be performed to suit the unique needs of the patients being served and the unique requirements for each sponsor.

The RCC is NOT designed to be a one-size-fits-all solution. The RCC is designed to flexible and scalable to meet the unique needs of each program. If required, the RCC is fully compliant with all CMS Part D requirements. However, the RCC can be scaled up to meet additional requirement desired by program sponsors and can be scaled back to meet more modest requirements if that is desired by a program. Data can be added and removed in minutes from the data fields and tracked immediately in reporting. Specialty clinical endpoints can be added for patients who have unique needs. In short, the RCC is sized and configured to meet a program sponsors needs and not the other way around.
